In order to improve performance in the serial modules and add DB constraints,
this patch is going to add missing foreign key on the following columns:
* serial.biblionumber
* serial.subscription
* subscriptionhistory.biblionumber
* subscriptionhistory.subscriptionid
* subscription.biblionumber
Once done, some code can be removed from the Del* subroutines, as the ON
CASCASE clause will make the RDBMS handles the deletions.
Test plan:
0/ It would be useful to test the update DB entry on a big and old
production DB, to make sure the constraints will be added correctly.
We could remove the entries before creating the constraints, but it can
be unecessary
1/ Make sure you can recreate a fresh install with the kohastructure.sql
from this patch
2/ Make sure you can upgrade from a master install
3/ Create a subscription, serial, etc. and delete the biblio
=> The subscription/serials should have been deleted from the DB
4/ Create a subscription, serial, etc. and delete the subscription
=> The serials should have been deleted from the DB

- Removes table printers
- Removes branchprinter column from branches
Check that:

Column created_on should not be updated. It is filled once by the
store method of Koha::ArticleRequest.
Column updated_on should be a timestamp, updated by the database.
Note: Although higher versions of MySQL and MariaDB support two timestamps

created_on column.

The following 2 fields are never used in the statistics table:
associatedborrower
usercode
This patch removes them.
To test:

accountlines.accounttype is currently a short varchar(5) field for to
store accounttype codes. Koha is moving towards more descriptive codes
which require a larger capacity varchar(16).

This patch adds a new column to the categories table:
'reset_password' which is a boolean, and represents, well... that. The
value itself is nullable.
The idea is that OpacResetPassword governs the general behaviour, and
this can be overriden at category-level.

Test plan:
1. Apply patch and update database
2. Verify that you can add a search field mapping for both 100a and 100A.
3. Verify that the above also works with a newly-created database.

The existing pattern is:
DROP TABLE IF EXISTS ...
CRATE TABLE
Stockrotation was using:
CREATE TABLE IF NOT EXISTS ...
which would not recreate the table in a database it's already
existing in possibly causing mismatches in table structure.
To test:
- Drop your database
- Run through the web installer
- Make sure there are no Database/SQL errors
- Makse sure the 4 stockrotation tables have been created

This patch change renewals column of issues and old_issues tables.
Now it is NOT NULL default 0. It does not make sense when number of
renewals is undefined, it is always integer with value zero or more
