Prior to perl 5.12 keys can only operate on a hash. So although
$data[0] ( thats an abysmal variable name! ) will contain a hash ref
the perl compiler cannot deduce that from the context and gives
a syntax error. Add the hash sigil to make the context explicit and
the compiler can generate the correct code.

i've discovered a bit late - a series of patches to fix issues for the 'ajax circ' feature (BZ11703) in 3.16
http://bugs.koha-community.org/bugzilla3/show_bug.cgi?id=11703
specifically, these patches...
730f416 Bug 13537 - Focus should move to barcode after renewing via checkouts table
2d70457 Bug 13290 - Add better feedback for returns to the checkouts table
7477cbc Bug 13408: Tell librarians they may continue scanning while checkouts table is loading
24a749d Bug 13293 - Regression: Override renewal limit option broken with AJAX circ
6e20fdf Bug 13286 - Checkouts table due date not updated when an item is renewed.
13fda83 Bug 13195 - Regression: Circulation checkouts table no longer shows item type description
48db11d Bug 13236 - Regression: Table of checkouts no longer preselects overdue items for rewewal
17e6a8b Bug 12778 - Regression: Item lost status doesn't show in list of checkouts
dd385fc Bug 13253 - Unnecessary white space above checkouts table in circulation
8848e72 Bug 13020 - Checkouts table default ordering is incorrect
d8ed913 Bug 12899: Row grouping in checkouts table is alphabetical and depends on translation
b8fae84 Bug 12729 - Overdue items won't show as overdue in red in circulation
most of these patches don't apply clean, and need to be manually added to 3.16 (which is also risky and fiddly)
because these patches are (imho) the most urgent for 3.16 - i plan to reset the 3.16.x branch back to the 'v3.16.6 tag', then add these corrected patches 1st, before less urgent patches

This patch adds the first name and surname of the librarian and the
user involved in a transaction log, if available.
If neither the firstname or the surname is available, then the 'object'
number will be prefaced with the 'Member' qualifier.
_TEST PLAN_
0) This patch depends on Bug 11473, so you must apply it first
Before applying this patch (11477):
1) View the logs.
2) Note that the Librarian column just has a number
3) Note that the Object column will just say Member X (where X is a
number), in the circulation, fines, and patron/member modules.
Apply the patch.
4) Reload the logs (you don't need to make new ones)
5) Note that the Librarian and Object columns mentioned above now
have firstname and surname appearing (where available)

This patch adjusts the check on action_logs.info to better detect
if the event concerns an item. This fixes a problem where the links
from log entries on item modifications would, if followed, result
in internal server errors.

Test Plan:
1) Apply this patch
2) Run updatedatabase.pl
3) Enable patronimages
4) Verify patron images are still displaying correctly
5) Test deleting a patron image
6) Test adding a patron image from moremember.pl
7) Test adding a patron image from tools/picture-upload.pl

Adds utf8 cgi parameter decoding.

This is ok in this case, as we are not doing any post/get of binary
data, but I note from the man page
This makes CGI.pm treat all parameters as UTF-8 strings. Use this with
care, as it will interfere
with the processing of binary uploads. It is better to
manually select which fields are expected
to return utf-8 strings and convert them using code like
this:
use Encode;
my $arg = decode utf8=>param('foo');

The routing list tab displays on patron pages even if the RoutingSerials
preference is OFF. Display of the tab should be conditional on that pref
being turned on.
This patch adds a check for the RoutingSerials preference to the menu
include files and amends the affected scripts to make the variable
available on the pages where those includes are used.
To test, view the following pages with RoutingSerials both on and off.
The routing list tab should be shown and hidden accordingly:
- Circulation
- Patron details
- Patron fines
- Pay fines
- Pay amount/selected (click from the Pay fines page)
- Create manual invoice
- Create manual credit
- Patron circulation history
- Patron modification log
- Patron notices
- Patron routing lists
- Patron statistics
- Patron files
- Patron permissions
- Set patron password
- "Can't delete patron" page (try to delete a patron with checkouts).

Updated all scripts appearing on the tools page
to respect a granular permission defined for
each of them.
The tools menu and home page have been changed so
that only the specific tools that a user has
access are displayed. This is simple, but depending on
the module and circumstance, it may be better to
display functions that the user has does not have
access to, but disable the links and do some sort
of visual styling to indicate that a function exists
but requires additional privileges to access.

Rule :
- always format_date() before sending date to template
- always format_date_in_iso immediatly after reading a date coming from a parameter
- deal internally only with dates in iso
Note that :
- I could not test things related to issues stats due to the my DB -issues.issuedate not filled, see commit about 3.00.00.006
