Bug 37907: Add column to user summary to show date an item was checked out on 

This patch adds a "Checked out on" column to the table of checkouts
and the table of overdues on the OPAC user summary page.

To test, apply the patch and log in to the OPAC as a user who has
overdue items.

- On the user summary page confirm that the "Checked out" table shows a
  column for "Checked out on" and that the information is
  displayed correctly.
- Make the same check of the "Overdue" tab.
